Pricing

1. We submit the billing to your insurance company
2. You may be responsible for charges, such as co-insurance and deductible
3. You are responsible fro paying your co-pay before services are rendered.
4. Cash Rate:
   • $200 for the Initial Evaluation
   • $150 for subsequent visits
